Irregularities

Accessibility

Incorrect Resource or Accommodation Setting (For All Assessments)

A designated support or accommodation required by the student’s IEP or Section 504 plan was set incorrectly and needs to be changed.

Incorrect Designated Support (For the Summative ELPAC and Initial ELPAC Only)

A designated support for a student without an IEP or Section 504 plan was set incorrectly and needs to be changed.

Designated Language Support (For the CAASPP Only)

A student was given an incorrect designated support for a translation (stacked or glossary) resource, or the streamlined interface for a student without an IEP or Section 504 plan was set incorrectly.

Incorrect Domain Exemption

Incorrect Domain Exemption or Alternate Assessment (For the Initial ELPAC Only)

A student did not receive a domain exemption or alternate assessment as prescribed by the student’s IEP or Section 504 plan.

Incorrect Domain Exemption (For the Summative ELPAC Only)

A student did not receive a domain exemption as prescribed by the student’s IEP or Section 504 plan.

Administration Error

Scores for the Speaking Domain (For the Summative ELPAC Only)

A test examiner did not score the Speaking domain responses in real time and cannot determine scoring from voice capture.

Incorrect Directions for Administration (DFA) Used

The DFA for a different assessment or domain was used instead of the DFA required for the assessment or domain given.

No Video Monitoring During Remote Test Session (For the Initial ELPAC Only)

An Initial ELPAC that was started or completed by the student(s) resulted in the absence of ongoing, mandatory, remote test session video monitoring.

Remote Test Session Not Paused (For the Initial ELPAC Only)

A test examiner lost video or chat access during an ongoing Initial ELPAC domain-level assessment and did not pause the test.

Incorrect Test Session Type Selected (For the Initial ELPAC Only)

A test examiner incorrectly selected the test session type In Person instead of Remote or Hybrid to administer an Initial ELPAC.

Administered Incorrect Assessment

Student Ineligible (For the Initial ELPAC and Summative ELPAC Grades Three Through Twelve Only)

The student(s) is no longer eligible for the Initial ELPAC assessment because the student's ELAS or primary language has changed.

The existing ELAS status does not qualify the student(s) to be tested for a summative assessment.

Incorrect SSID Used

No Switch

Reset the test and allow the student to start a new test.

Switch

Associate the assessment results with the student’s correct SSID. Note: Select this option only if the correct and incorrect SSID(s) are known.

Student Issues

Student(s) Cheating or Providing Answers

Student(s) cheated or provided answers to each other, including passing notes and discussing test items with other students during testing.

Student(s) Accessing Internet or Unauthorized Devices or Software Applications

Student(s) accessed either the internet or any unauthorized software applications, or used an unauthorized electronic device (e.g., a cell phone, tablet, Bluetooth headphones or smartwatch), during the testing event.

Validity

Adult Interference (Allowing Nonprescribed Accommodation or Materials)

A test administrator or test examiner allowed inappropriate accommodations during test administration that are not designated in the student’s IEP or Section 504 plan, provided students with nonallowable materials or devices, or modified student responses.

Student Does Not Respond (For the CAASPP Only)

A student deliberately did not attempt to respond appropriately to items.
